If you are a patient transitioning to Inova 360 Concierge Medicine with us, there’s nothing you need to do. Your records will be transferred automatically once you’ve completed the enrollment process.

If you are not transitioning with us and would like to request medical records to be sent to your new primary care physician, please have your new PCP’s office fax a medical release request to (703) 790-8924.
Please allow 10-14 business days for request to be completed.
